Accident and Incident Investigation
When something goes wrong a thorough investigation can help identify the basic causes so that action can be taken to prevent a recurrence. Examples would be after a RIDDOR reportable accident, a significant incident or even a near miss. The investigation would gather relevant information for reference in the event of enforcement activity or a claim. Typically we would visit site and gather the facts, interview witnesses and produce a report with recommendations. If appropriate we will liaise with enforcement authorities.
